Needed Parents to Chaperone
The following list identifies your responsibilities as a chaperone when accompanying students on Duval County Public School field trips. Please review the list, and let me know your intentions by January 15, 2009. If you are interested in being a chaperone you must meet us either at MOSH or at Kirby Smith Middle School.
-Must be 21 years old or a parent of a student going on this field trip.
-No children except the ones going or participting on this trip.
-Provide close supervision to a group of under 10 students.
-A list if names in your group will be provided to you.
-No tobacco or alcholic beverages are premitted on this field trip.
-IMMEDIATELY report ANY PROBLEMS directly to the teacher.
-Arrive promptly for departure of trip.
